AFTER A FAMILY IS REFERRED, IT MUST COMPLETE AN APPLICATION AND SUBMIT A FORMAL INTAKE FORM WITH ACCOMPANYING DOCUMENTATION SUCH AS W-2 FORMS, INCOME TAX FILINGS, PAYCHECK STUBS, INFORMATION ON SOCIAL SECURITY AND DISABILITY AWARDS, PUBLIC AID DOCUMENTS, AND LIVING EXPENSE INFORMATION. THE CHILDREN MUST PROVIDE GRADE REPORTS. |
